We are currently undertaking a project to evaluate and improve the experience of young people and families as they transition from children's to adults' services provided by Berkshire Healthcare Foundation Trust. We have received informal feedback via the Parent Carer Forums across the west of the county and as a result, we would like to gather the views of as many service users as possible. This valuable feedback will help to inform our service development and enable us to identify any changes that need to be made. We would like to gather the views of young people and their families who are aged between 18-20, so have recent experience of the transition process.

We have worked with representatives from the Parent Carer Forums to devise a questionnaire, which we would be most grateful if you could fill in. We anticipate that this will take between 5 and 15 minutes. There are two versions of the questionnaire. One is an 'Easy Read' version supported by symbols and pictures which can be completed by young people with learning disabilities, with or without support from a carer as appropriate. The other is to be completed by parents and carers. Ideally we would find it helpful if you could fill in both of these. We are really interested to hear the view of the young person as well as the carer.

We are mindful that this request comes to you at a particularly busy and challenging time as we all juggle the demands of work and family during a pandemic. We had identified this project was a priority for us prior to this recent lockdown and we were keen to avoid the pandemic causing a delay to the improvement work. We hope that you are able to find the time to support us with this.
